1. Our Commitment to Your Privacy

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. At Exline Health LLC, we are required by law to maintain the privacy of your Protected Health Information (PHI) and your substance use disorder (SUD) records.

2. Special Protections for SUD Records (42 CFR Part 2)

Because we provide substance use disorder services, your records receive extra protection under federal law. We will not disclose that you attend this program or share any information identifying you as having a substance use disorder unless:

  1. You provide written consent (including the "single consent" for treatment, payment, and operations).
  2. A court issues a specific order meeting Part 2 requirements.
  3. The disclosure is otherwise permitted by 42 CFR Part 2 (e.g., medical emergency, child abuse reporting).

3. Maine Health Information Exchange (HealthInfoNet)

Exline Health LLC is currently in the process of connecting to HealthInfoNet, Maine’s statewide Health Information Exchange (HIE).

  • Our Planned Participation: We plan to begin sharing health information with HealthInfoNet in the near future to better coordinate your care with other Maine providers.
  • SUD Records are "Opt-In" Only: Even once we are connected, your substance use disorder records will never be shared with HealthInfoNet unless you sign a specific "Opt-In" authorization.
  • General Health Opt-Out: You have the right to "Opt-Out" of the exchange entirely at any time. If you opt out, your medical information will not be viewable by other providers through the HIE, even in an emergency.

4. How We Use and Disclose Your Information

Under the 2026 revised standards, we use and disclose your records for:

  • Treatment, Payment, and Operations (TPO): With your single written consent, we may share information with your other doctors, your insurance company, or our internal billing/quality teams.
  • Mandatory Reporting: We are required by Maine Law (22 M.R.S. § 4011-A) to report suspected child abuse or neglect.
  • Safety Threats: We may disclose information to prevent a direct threat of imminent harm to yourself or others.
  • Legal Proceedings Protection: Your records cannot be used in any civil, criminal, or legislative proceedings against you without your specific consent or a court order.

5. Your Rights

  • Right to Access: You may inspect or get a copy of your records (usually provided within 30 days per Maine Law 22 M.R.S. § 1711-B).
  • Right to Amend: You may ask us to correct errors in your record.
  • Right to Accounting: You may request a list of certain disclosures we have made of your PHI.
  • Right to Revoke: You can withdraw your consent for future disclosures at any time.
